A BILL
To amend the Child Nutrition Act of 1966 to establish
a community health partnership grant program.
Be it enacted by the Senate and House of Representa-

tives of the United States of America in Congress assembled,

SECTION 1. SHORT TITLE.
3
This Act may be cited as the ‘‘Community Access,
4
Resources, and Education for Families Act of 2020’’ or
5
the ‘‘CARE for Families Act of 2020’’.
6
SEC. 2. COMMUNITY HEALTH PARTNERSHIP GRANTS.
7
Section 17(h) of the Child Nutrition Act of 1966 (42
8
U.S.C. 1786(h)) is amended by adding at the end the fol-
9
lowing:

‘‘(15)
COMMUNITY
HEALTH
PARTNERSHIP

GRANTS.—
2
‘‘(A) DEFINITIONS.—In this paragraph:
3
‘‘(i) ELIGIBLE ENTITY.—The term ‘el-
4
igible entity’ means—
5
‘‘(I) a local agency; and
6
‘‘(II) a local clinic.
7
‘‘(ii) PROGRAM
PARTICIPANT.—The
8
term ‘program participant’ means a partic-
9
ipant in the special supplemental nutrition
10
program under this section.
11
‘‘(B) ESTABLISHMENT.—Not later than 1
12
year after the date of enactment of this para-
13
graph, the Secretary shall establish a program
14
to make grants to eligible entities to enhance
15
collaboration among—
16
‘‘(i) the eligible entities and commu-
17
nity health partners, including health care
18
providers, social services programs, and
19
early childhood learning and care pro-
20
viders—
21
‘‘(I) to establish linkages between
22
the eligible entities and those part-
23
ners, such as through participation in
24
community coalitions;

‘‘(II) to facilitate referrals among
1
the eligible entities and those part-
2
ners;
3
‘‘(III) to implement evidence-
4
based strategies to improve the health
5
of communities, conduct outreach to
6
existing and potential program par-
7
ticipants, and promote breastfeeding
8
and access to healthy foods; and
9
‘‘(IV)
to
increase
awareness
10
among those partners and existing
11
and potential program participants of
12
the eligibility requirements for, and
13
health benefits of, the program, as ap-
14
plicable; and
15
‘‘(ii) the eligible entities and health
16
care providers such as pediatricians, obste-
17
tricians-gynecologists, family physicians,
18
advance practice nurses, nurse midwives,
19
community health centers, health depart-
20
ments, hospitals, and rural health clinics—
21
‘‘(I) to facilitate and improve ac-
22
cess to comprehensive prenatal, post-
23
natal, and postpartum care for pro-
24
gram participants;

‘‘(II) to facilitate certification of
1
eligibility of persons for participation
2
in the program and provision of pro-
3
gram benefits at the hospital bedside
4
for eligible postpartum women and in-
5
fants;
6
‘‘(III) to improve the coordina-
7
tion, quality, and cost effectiveness of
8
health care services;
9
‘‘(IV) to improve the sharing of
10
information necessary to establish nu-
11
tritional risk; and
12
‘‘(V) to ensure consistent nutri-
13
tion education and breastfeeding mes-
14
sages are provided to program partici-
15
pants.

‘‘(C) APPLICATION.—To be eligible to re-
17
ceive a grant under this paragraph, an eligible
18
entity shall submit an application to the Sec-
19
retary at such time, in such manner, and con-
20
taining such information as the Secretary may
21
require.

‘‘(i) IN GENERAL.—Of the funds au-
1
thorized to be appropriated under sub-
2
section (g)(1)(A), the Secretary shall use
3
to carry out this paragraph $15,000,000
4
for each of fiscal years 2020 through
5
2025.
6
‘‘(ii)
ADJUSTMENT.—The
amount
7
specified in clause (i) shall be adjusted an-
8
nually for inflation by the same factor used
9
to determine the national average per par-
10
ticipant grant for nutrition services and
11
administration for the fiscal year under
12
paragraph (1)(B).’’.
