Be it enacted by the Senate and House of Representa- 1 tives of the United States of America in Congress assembled, 2 SECTION 1. FUNDS FOR COMMUNITY HEALTH PARTNERSHIP 1 OUTREACH. 2 Section 17(h) of the Child Nutrition Act of 1966 (42 3 U.S.C. 1786(h)) is amended by adding at the end the fol- 4 lowing: 5 ‘‘(15) COMMUNITY HEALTH PARTNERSHIPS 6 GRANTS.— 7 ‘‘(A) PROGRAM ESTABLISHED.—Not later 8 than 1 year after the date of the enactment of 9 this paragraph, the Secretary shall carry out a 10 grant program to make grants to local agencies 11 and clinics to carry out activities to enhance 12 collaboration between— 13 ‘‘(i) such local agencies and clinics 14 and community health partners, including 15 other health care providers, social services 16 programs, and early childhood learning 17 and care providers, for the purposes of— 18 ‘‘(I) establishing linkages be- 19 tween such entities, such as through 20 participation in community coalitions; 21 ‘‘(II) facilitating referrals be- 22 tween such entities; 23 ‘‘(III) implementing evidence- 24 based strategies to improve the health 25 of communities, conduct outreach to 26 VerDate Sep 11 2014 00:48 Jun 15, 2019 Jkt 089200 PO 00000 Frm 00002 Fmt 6652 Sfmt 6201 E:\BILLS\H3117.IH H3117 pamtmann on DSKBFK8HB2PROD with BILLS 3 •HR 3117 IH eligible participants, and promote 1 breastfeeding and access to healthy 2 foods; and 3 ‘‘(IV) increasing awareness 4 among such entities and program par- 5 ticipants (including potential program 6 participants) of the eligibility require- 7 ments for, and health benefits of, the 8 program; and 9 ‘‘(ii) such local agencies and clinics 10 and other health care providers, including 11 pediatricians, obstetricians-gynecologists, 12 family physicians, advance practice nurses, 13 nurse midwives, community health centers, 14 health departments, hospitals, and rural 15 health clinics, for the purposes of— 16 ‘‘(I) facilitating and improving 17 access to comprehensive prenatal, 18 postnatal, and postpartum care for 19 program participants; 20 ‘‘(II) facilitating certification of 21 eligibility of persons for participation 22 in the program and provision of pro- 23 gram benefits at the hospital bedside 24 VerDate Sep 11 2014 00:48 Jun 15, 2019 Jkt 089200 PO 00000 Frm 00003 Fmt 6652 Sfmt 6201 E:\BILLS\H3117.IH H3117 pamtmann on DSKBFK8HB2PROD with BILLS 4 •HR 3117 IH for eligible postpartum women and in- 1 fants; 2 ‘‘(III) improving the coordina- 3 tion, quality, and cost effectiveness of 4 health care services; 5 ‘‘(IV) improving the sharing of 6 information necessary to establish nu- 7 tritional risk under subsection (b)(8); 8 and 9 ‘‘(V) ensuring consistent nutri- 10 tion education and breastfeeding mes- 11 sages are provided to program partici- 12 pants. 13 ‘‘(B) APPLICATION.—To be eligible to re- 14 ceive a grant under this paragraph, a local 15 agency or clinic shall submit an application to 16 the Secretary at such time, in such manner, 17 and containing such information as the Sec- 18 retary may require. 19 ‘‘(C) AUTHORIZATION OF APPROPRIA- 20 TIONS.— 21 ‘‘(i) IN GENERAL.—From amounts 22 appropriated under subsection (g), 23 $15,000,000 may be made available to 24 VerDate Sep 11 2014 00:48 Jun 15, 2019 Jkt 089200 PO 00000 Frm 00004 Fmt 6652 Sfmt 6201 E:\BILLS\H3117.IH H3117 pamtmann on DSKBFK8HB2PROD with BILLS 5 •HR 3117 IH carry out this paragraph for each of fiscal 1 years 2020 through 2025. 2 ‘‘(ii) ADJUSTMENT.—The amount 3 specified in clause (i) shall be adjusted an- 4 nually for inflation by the same factor used 5 to determine the national average per par- 6 ticipant grant for nutrition services and 7 administration for the fiscal year under 8 paragraph (1)(B). 9 ‘‘(D) DEFINITIONS.—In this paragraph: 10 ‘‘(i) PROGRAM.—The term ‘program’ 11 means the special supplemental nutrition 12 program under this section. 13 ‘‘(ii) PROGRAM PARTICIPANT.—The 14 term ‘program participant’ means a partic- 15 ipant in the special supplemental nutrition 16 program under this section.’’. 17 Æ VerDate Sep 11 2014 00:48 Jun 15, 2019 Jkt 089200 PO 00000 Frm 00005 Fmt 6652 Sfmt 6301 E:\BILLS\H3117.IH H3117 pamtmann on DSKBFK8HB2PROD with BILLS
